Qualifications:
- I am a CRNA working in a rural Washington State community. My goal is to improve my own practice and the overall care given at the facility in which I am employed. I plan to complete my doctorate degree and utilize the unique rural anesthesia practice that I work in as a platform for increasing understanding and knowkege in rural anesthesia practice.
- I have been involved in critical care medicine since 1993. I began as an EMT volunteering with a small but busy ambulance agency and advanced myself at every opportunity, eventually becoming a flight nurse with a busy and well respected air medical service. I studied diligently throughout my career and maximized each opportunity. I took each of the critical care specialty certification examinations in the areas that I’ve worked and have maintained those certifications. I have a Masters degree as a Family Nurse Practitioner and a second Masters degree as a Nurse Anesthetist. I have a great deal of education as well as experience and will be an asset to any anesthesia practice.
Experience:
- September '08 to present. Mid-Valley Hospital, Omak, Washington. Full time CRNA in the anesthesia department. I provide anesthesia for the hospital in the OR, OB and ER throughtout the week and call every third weekend.
- July ’05 to Aug ‘06. Utah Valley Regional Medical Center, Provo, Utah. Emergency Room. Full time staff nurse and team leader in the ER of a Level 2 trauma center. I cared for a variety of critically ill or injured adults and pediatrics. Because of my experience and education, I was often chosen to care for trauma and cardiopulmonary arrest situations. I educated students and trained newly hired RN’s for the emergency department. On occasion I managed the department as team leader. I was involved in many emergency procedures and was allowed to perform Arterial lines.
Near the completion of my Nurse Practitioner program I was able to work for the ER physician group and manage the “fast track” of the emergency room. I was able to interview and assess patients, order and evaluate tests and x-rays, and perform procedures such as I&D’s, uncomplicated skin closures, and fracture reductions, pleural and peritoneal fluid evacuations. I was also able to perform central line placements, and intubations. I ordered medications and wrote prescriptions.
- July ’03 to July ‘05. Utah Valley Regional Medical Center, Provo, Utah. Medical/Surgical Intensive Care Unit. Full time staff nurse in the medical surgical ICU. I cared for critically ill and injured patients in either the surgical unit or the medical unit. I frequently cared for septic patients with a variety of diseases, common and obscure. I cared for two Leptosporosis patients, an occasional hantavirus patient, and a multitude of antibiotic resistant disease processes. I recovered open heart surgery patients, and frequently cared for patients with severe trauma.
- March ’02 to April ’03. University of Utah, SLC, Utah. Staff Nurse. Emergency Department. Per Diem status. I worked once or twice per week at this level 1 trauma status and burn facility. I frequently cared for patients flown in via air medical transport for a variety of pathologies such as illnesses, burns, and trauma. I was frequently asked to take the complicated trauma cases coming to the facility due to my experience and training. I worked closely with the ER attendings and assisted in teaching medical students and residents.
- May ’01 to present. Classiclifeguard Aeromedical, Utah/Arizona. Flight Nurse. Search and Rescue/NVG trained. I worked 1-3 shifts per month for 72 hours at a time and ranged anywhere from 4-14 flights per shift. I worked primarily rotor but spent one summer transporting patients long distances on a Lear Jet domestically and internationally. I cared for a variety of acutely ill or injured/burned adult, pediatric, infant, and obstetric patients. It was very common to fly injured or drowned patients from Lake Powell. We covered a great deal of the Navajo Nation in northern Arizona and it was not uncommon to fly into the Grand Canyon or any of the many canyon lands in the area for a search and rescue. I worked many cardiopulmonary arrest situations and I performed a variety of critical care skills.
Upon my acceptance to Anesthesia school I became the web site administrator (June ‘06: www.classiclifeguard.com). I continue to assist in education and administration and as a resource for my colleagues. I design internet based exams and quizzes for continuing education, design and build web pages, maintain employee email and scheduling, billing, and an interdepartmental communication blog.
- March ’98 to July ‘03. Timpanogos Regional Hospital, Orem, Utah. Staff/Charge Nurse. Emergency Department. I was charge nurse and worked primarily night shift. Opened new ER facility and interfaced with EMS and assisted in establishing protocols and processes for the department and EMS. Primary nurse in Code Blue Team for the facility while at work.
- September ’96 to March ’98. Brigham City Community Hospital, Brigham City, Utah. Staff Nurse. Emergency Department. I was charge nurse and worked both day and night shifts while attending university for my BSN degree at Weber State University. I cared for a variety of acutely ill and injured. Cared for a high number of trauma and burn cases due to location of several large industrial facilities such as Nucor Steel, Autoliv, Morton-Thiokol, and others. There were a large number of freeway accidents and trauma transported via EMS from I-15 and Sardine Canyon. Limited staff and limited personnel resources required me to run codes and manage trauma independently using EMS and other in-hospital staff as the ER physician was also independently running cardiopulmonary arrest and trauma situations.
- December ’94 to September ’96. Sanpete Valley Hospital, Mt. Pleasant, Utah. Staff Nurse. ED/MedSurg/PACU. Worked as LPN initially and then transitioned to RN. Along with 1 RN and 1 CNA managed the medical surgical unit and the Emergency Room at night and added 1 RN and CNA during the day for post surgical unit management and obstetrics when needed. I was able to have a wide variety of experience while working at this small facility, stabilizing trauma and severely ill patients for transport via helicopter.
- June ’93 to January ’95. EMT-I with Ephraim Ambulance Association. Cared for a wide variety of medical emergencies and trauma from vehicle, equestrian, ATV, snowmobile and other various accidents. I specialized as an advanced EMT with additional courses in utilizing defibrillators and AED’s and Military Anti-Shock Trousers (MAST). I had the opportunity to be involved in various search and rescue operations.
Education:
- Master of Science in Nursing, Nurse Anesthetist, California State University Fullerton, Kaiser Permanente School of Anesthesia; August ’06 to Aug. 15, 2008. I currently have a 3.84 GPA. I had the opportunity of rotating through several challenging specialty rotations. I worked at Children’s Hospital of Los Angeles for my pediatric rotation, Kaiser Permanente Sunset Hospital in Los Angeles for open heart, KP Anaheim Medical Center for neurosurgical anesthesia, and Navy Medical Center in San Diego for obstetrics and regional anesthesia.
I also worked in a wide variety of general surgical anesthesia rotations including the Loma Linda VA Hospital and Loma Linda University Medical Center. Other rotations included Riverside County Hospital, Riverside Kaiser Hospital, California Hospital and UCLA Medical Center both in Los Angeles, and St. Mary’s Medical Center in Apple Valley. I am fluent in Spanish and I have used it extensively throughout my clinical rotations as there is a large Latin population in this area.
- Master of Science in Nursing, Family Nurse Practitioner, University of Utah, Salt Lake City, Utah; May ’04 to May ’06. I graduated with a 3.83 GPA. I rotated through a variety of clinical settings in Utah such as nighttime pediatrics and instacare in Stansbury Park, Teen Mom at the University of Utah, Internal medicine in West Valley and Veterans Administration Clinic in Orem, Orthopedics in Provo, Payson and Nephi. I made arrangements for a specialty rotation with Pulmonology and Intensive Care at Utah Valley Regional Medical Center and Emergency Medicine at the same facility where I also managed the Fast-Track area independently. My synthesis/thesis was titled “Syncope Evaluation; Discussion and Guidelines for the Syncope Workup in Young Adults” and was evaluated for publication by the University of Utah Graduate Studies Committee in May 2006.
- Bachelor of Science in Nursing, Weber State University, Ogden, Utah; January ’96 to August ’97. Graduated with a 3.62 GPA. I was re-elected class president and participated in various administrative activities for the class and program. I worked full time in the Emergency Room at Brigham City hospital while attending University, as well as rotating through a variety of clinical experiences such as the Intensive Care Unit of Ogden Regional Hospital, the Ogden homeless shelter, an inpatient psychiatric center in Salt Lake City, and an elective rotation in the Emergency Room in Grand Junction, CO with a Nurse Practitioner while preparing to apply to a Nurse Practitioner Program.
- Associate of Science in Nursing, Weber State University, Ogden, Utah; September ’95 to August ’96. I worked full time at Sanpete Valley Hospital while attending University. I was elected as class president and participated in various administrative and planning activities and committees.
- Associate of Science, Snow College, Ephraim, Utah; June ’93 to June ’95. I worked full time doing various jobs as well as Ephraim Ambulance building experience and education in order to enter a nursing program.
Industry Involvement / Certifications / Licensure / Conferences:
- I attended the San Diego Anesthesia Symposium May 12-13th 2007. I subscribe to the AANA journal and I am a current member of the AANA. I am a board certified Family Nurse Practitioner with certification from the American Nurses Credentialing Center (ANCC) and I have a current license as a Family Nurse Practitioner in California as well as a RN license in both California and Utah. I currently have certifications as a Certified Emergency Nurse (CEN), Critical Care Registered Nurse (CCRN), and Certified Flight Registered Nurse (CFRN). I am current with Basic Life Support, Advanced Cardiac Life Support, and Pediatric Advanced Life Support.
Additional Sections:
- I am very family oriented and enjoy helping others. I served as a missionary for two years in Puerto Rico helping the local people there improve their lives and I became fluent in Spanish as a result. I enjoy the country and a variety of activities including mountain biking, snowboarding, camping, and other outdoor activities.
- I feel that I am well suited to smaller cities and towns and I hope to incorporate both of my clinical specialties into a practice. My wife is also a nurse practitioner and looks forward to working and using her skills in some capacity within the community. I am very good with computers and I am always eager to incorporate new technologies and advancements. I have always enjoyed learning and I have a goal of beginning a clinically oriented doctorate degree in the coming years.
